System Navigation
We provide patients with two or more permanent health conditions with intensive support in navigating the healthcare and social services system. We provide these families with a single window of service to navigate programs across the Ministry of Health, Ministry for Seniors, Ministry of Long Term Care, Ministry of Children, Community and Social Services, ServiceOntario, and community agencies.
Health Education
In collaboration with our community partners, our series of best-in-class webinars, seminars, and workshops educates patients and families with long-term conditions on topics important to their health and social care - from system information to advance directives and powers of attorney. Dedicated Support for Healthcare Providers
We understand that community healthcare providers are in the business of providing excellent healthcare to patients, not navigating systems. We can support your providers directly with information, advice, navigation support, access to our library of resources, and connections to our network of partners!
