Our Patient Navigators are available across North America to act as your point of contact, , provide referrals, resources, support access to services and care plans to support you in getting back to managing your own health, we believe you are your own best advocate!
Our Shining Star Program, Valued Connections Healthcare is dedicated to helping you learn to manage your own healthcare. We deliver a variety of Community Seminars, Webinars and Lunch and Learns - both In Person and Virtually. We even offer some with Community Partners. You are your own best advocate!
SSAH, ACSD, ODSP, SSI, OAP - These acronyms are confusing in their self. You have enough on your plate. Being a bookkeeper should not be! Our Direct Funding Accounts Coordinators take care of everything.

09/27/2023. Masking is now mandatory in Patient Care Areas and when working with Patients for our Staff, Learners, and Volunteers. Patients and Visitors are encouraged but not required to wear a mask. Non-Clinical and Public Space remains mask friendly.
