Navigation Help Centre
Our Flagship Program! We support patients and families in navigating and managing all Health and Social Services needs Post Acute Care, or In the Community.
Our Team can support you with financial assistance, arranging medical travel, accessing Home and Community Care Support Services, arranging Doctor's Appointments and MORE!
Health Education
Valued Connections Healthcare is dedicated to helping you learn to manage your healthcare.
We deliver a variety of Community Seminars, Webinars, and Lunch and Learns - both In Person and Virtually. We also come to community events and even carnivals - healthcare can be fun!
We also offer some with Community Partners. You are your own best advocate! Have a topic you want to see - email Stakeholder Experience!

Financial Assistance Program
Healthcare is expensive - and it should not be, that is why our Program offers assistance towards Uninsured Medically Required Costs. These programs are not available to ODSP and OW Clients.

Medical Travel
We offer direct-to-vendor subsidies based on defined eligibility requirements to assist with Taxi, Fuel and Public Transporation to Medical Appointments.

Prescription Medication
We assist with the costs of medication for those not eligible for OHIP+ / ODB OR require medication where there is no OHIP Funded Option that is Therapeutically appropriate.
We assist with the costs of medication for people who are unable to access OHIP + or ODB - and do not have private insurance.
We also may assist when the patient requires a medication not covered by OHIP and there is no clinically appropriate alternative. Patients must have applied for the Exceptional Access Program FIRST.

Fee for Service Programs
Support for Children, Youth, and Adults with Complex Needs. Services are eligible for Passport Funding.

Person Directed Planning
We support patients and families in navigating the service system, accessing services and creating coordinated care plans that allow patients to lead there own care.
We also support accessing and applying for all funding.

Funds Trusteeship & Brokerage
We are able to manage Transfer Payment Funding. No need to submit invoices - simply email us your receipts and we will take care of the rest. Issuing you one payment for all funds from the previous month on the 16th of the month!
We broker the following
- Special Services at Home (SSAH)
- Ontario Autism Program Funding (OAP)
- the Passport Program
We provide Ministry Trusteeship for the following programs - The program is not endorsed by the Ministry. Client must access other programs
- Ontario Works, Social Assistance & Employment Opportunities
- Ontario Disability Support Program
