Are there any fees for service?
No! All programs are free of charge at the point of service - excluding transfer payment management. Which is at 10% of the total funds (This is deducted from the funds and not out of pocket)
Am I eligible for services?
Each Program has its own eligibility criteria, and programs can vary by area and funding. Please give us a call, and a Family Service Associate would be pleased to assist you in accessing the best programs for your family.

How do I pay an invoice?
Valued Connections Healthcare's core programs are available free of charge. This information is for fee-for-service (Passport), corporate sponsorships, space rentals, and ancillary revenue invoices.
Note: Cheque, Bank Draft, or Money order is preferred for individuals. EFT/Wire is preferred for Businesses. Thank you!

How do I obtain a copy of my health records?
If you received services at Valued Connections Healthcare and a file was opened for you (Case Management or Community Workshops), you may access your records in the following ways:

Return the form via FAX to 905 667 9476, Attn: Release of Information, or mail Attn: Release of Information - VCH, EON Environmental Corporate Support Site, 4999 Victoria Ave, Niagara Falls, ON, L2E 4C9. Please include a cheque or money order for the administrative fee.
Please note that both an administrative fee and a per-page fee will apply ($35 + $0.45/page) in accordance with industry standards. Please include the administrative fee via cheque or money order - a bill for the remaining balance will be provided upon completion of your request. This fee does not apply to Healthcare Providers.
Should you have any questions, please call the Support Centre at 1-877-369-8242 and use option 5 to leave a voicemail for support. Client Services or your family service worker does not have access to complete or comment on release of information requests.
